What is P-Max™?
P-Max is not a standalone treatment — it is a combination protocol that brings together two coordinated elements: an immediate girth component (P-Thick, or an HA filler placed with the UroSculpt technique) and the six-month P-Long regenerative protocol. Together they address both dimensions of enhancement at once.
The result is a protocol that delivers something most enhancement options cannot: immediate visible girth improvement from the first session, alongside a six-month program driving progressive length and girth development through biological tissue change.
The P-Long component of P-Max is entirely autologous — your own blood, PRP, and biology. The immediate-girth component may also be autologous (P-Thick) or may use an HA filler product, depending on which option you and your provider select. Your provider can walk you through the tradeoffs of each.
An immediate-girth treatment alone (P-Thick or HA filler) gives visible results right away but is temporary and doesn’t address length. P-Long alone gives gradual length and girth but takes months and requires daily participation before any visible change.

The immediate-girth portion of P-Max uses either thermally processed autologous PRP (P-Thick) or an HA filler, placed using the UroSculpt injection technique. Either option provides visible girth enhancement from the first session; your provider will discuss which material fits your goals, budget, and preference for autologous vs. filler material.
A structured six-month regenerative program combining PRP injections, daily traction, vacuum pump therapy, and nitric oxide supplementation to drive gradual biological tissue expansion.
On the material choice: UroSculpt refers to the injection technique used for the filler-based option, not a specific filler product. How the two phases work together
P-Max is structured in two phases that run in parallel — not sequentially. The immediate enhancement and the regenerative protocol begin together and progress simultaneously.
Immediate Enhancement
Your chosen immediate-girth material — DermaPRP (P-Thick) or an HA filler via the UroSculpt technique — is injected during the first session, providing immediate girth enhancement. This gives visible results from day one while the regenerative protocol begins to take effect over the following months.
Approximately 1 week of downtime follows the first session. This is the only significant recovery period in the protocol.
Regenerative Growth
The P-Long protocol begins alongside Phase 1 and runs for approximately six months. PRP injections are administered across 6 sessions spaced roughly 4 weeks apart. Daily traction and pump therapy continue throughout, building gradual length and sustained girth improvement.
Patient compliance with daily device use is the primary driver of P-Long outcomes. Results develop progressively — most men see meaningful change at the 3–4 month mark.

How much downtime does P-Max require?
The only significant downtime is approximately one week after the first session, related to the immediate-girth injection. Subsequent PRP sessions within the P-Long protocol typically have minimal to no downtime. Daily home protocol (traction and pump) continues throughout but does not require time off work or activity restrictions.
When will I see results?
The immediate-girth component (P-Thick or HA filler) delivers visible girth enhancement after the first session. The P-Long component builds gradually — most men notice meaningful length and girth changes at the 3–4 month mark, with continued improvement through the full six months. Full results are assessed at protocol completion.
How long do P-Max results last?
If you chose P-Thick, that girth enhancement lasts approximately 9–12 months before naturally reabsorbing. If you chose HA filler, longevity depends on the specific product used — ask your provider. The regenerative changes from P-Long may be longer-lasting, as they reflect structural tissue change rather than temporary volume. Long-term maintenance data is still evolving. Why does nicotine disqualify candidates?
P-Long relies on healthy blood flow, tissue oxygenation, and cellular regeneration. Nicotine — including tobacco, vaping, and nicotine replacement products — and marijuana impair circulation and oxygen delivery, directly undermining the mechanisms the regenerative component depends on. Providers typically require nicotine and marijuana cessation before and throughout the protocol.
Can P-Max be combined with shockwave therapy or TRT?
Yes. Shockwave therapy is commonly added to support vascular health alongside the regenerative components. Testosterone replacement therapy (TRT) may also be considered as part of a comprehensive men’s health plan. Any additions to the protocol should be discussed with and managed by your provider.
