How it works
Mechanism and what to expect
Sildenafil inhibits PDE5 — the enzyme that breaks down cyclic GMP in penile smooth muscle. When arousal occurs and nitric oxide is released, cGMP builds up and smooth muscle relaxes, allowing blood to flow into the corpus cavernosum. Sildenafil prolongs and enhances this response by preventing cGMP from being broken down prematurely.
The result is a firmer, more reliable erection in response to sexual stimulation. Sildenafil does not cause erections independently — arousal must be present. This is one of the most important things men don’t understand before their first dose.
Peak plasma concentration occurs 30–60 minutes after taking sildenafil on an empty stomach. A high-fat meal before taking it can delay absorption by 30–60 minutes and reduce peak concentration — which is why taking it on an empty stomach or after a light meal is recommended for best results.
A high-fat meal (burgers, fried food, heavy cream sauces) slows gastric emptying — which significantly delays sildenafil absorption. This is the most common reason men say “Viagra didn’t work” after taking it before a dinner date. Take it on an empty stomach or after a light snack for best results.
Alcohol can also reduce effectiveness and increase side effect risk — particularly flushing and blood pressure effects. Moderate alcohol (1–2 drinks) is generally fine; heavy drinking is not.
Never combine sildenafil with nitrates — including nitroglycerin, isosorbide, and some recreational drugs (poppers/amyl nitrate). The combination causes potentially life-threatening blood pressure drops. If you take any nitrate medication, sildenafil is contraindicated. Tell your physician about all medications before starting.
Dosing
Understanding the three doses
Sildenafil is available in 25mg, 50mg, and 100mg. Most men start at 50mg. The dose is adjusted based on response and side effects.
For men who are sensitive to medications, older men, or those with mild ED. Sometimes prescribed for men on multiple other medications.
Where most men start. If response is insufficient, dose can be increased to 100mg. If side effects are significant, reduce to 25mg.
The maximum recommended dose. Stronger effect — but also stronger side effects. Don’t increase to 100mg without trying 50mg first and discussing with your provider.

Common and important side effects
Side effects are dose-dependent — they are more common and more pronounced at higher doses.
Headache — most common; typically mild and transient
Facial flushing — warmth or redness, usually brief
Nasal congestion — stuffy nose lasting a few hours
Visual disturbances — mild blue tinge or light sensitivity; rare at standard doses
Indigestion — mild GI discomfort in some men
Blood pressure drop — especially with nitrates, alcohol, or alpha-blockers
Take it 45–60 minutes before planned activity. Empty stomach or light snack only. Avoid heavy drinking. Allow adequate time and stimulation — some men rush to evaluate the medication before it has fully worked. Give it 3–4 attempts before concluding it doesn’t work for you.
If 50mg produces side effects but insufficient response, discuss with your provider whether Cialis (tadalafil) might be a better fit — it has a different side effect profile and no food interaction.

Why didn’t Viagra work for me?
The most common reasons: taken with or after a heavy meal, insufficient time allowed before activity, inadequate sexual stimulation (it requires arousal), or the dose wasn’t right. Try taking 50mg on a genuinely empty stomach, allow 60 minutes, and ensure arousal is present before evaluating. If you’ve tried 3–4 times under optimal conditions and it still doesn’t work, discuss trying 100mg or switching to tadalafil with your provider.
Is generic sildenafil as good as Viagra?
Yes. Generic sildenafil contains the identical active ingredient at the identical dose. The FDA requires generics to demonstrate bioequivalence to the brand. Generic sildenafil can cost 95% less than brand Viagra. There is no pharmacological reason to choose brand over generic unless there is a specific formulation difference your provider recommends.
Can I take sildenafil with alcohol?
Moderate alcohol (1–2 drinks) is generally compatible with sildenafil. Heavy alcohol consumption increases the risk of blood pressure drop, reduces the efficacy of the medication, and can independently impair erectile function. Avoid combining sildenafil with heavy drinking for both safety and effectiveness reasons.
Can Viagra be combined with other ED treatments?
Yes — sildenafil is commonly used alongside shockwave therapy, PRP (P-Shot), and TRT. Using medication during a course of regenerative treatment is standard practice. Do not combine with nitrates under any circumstances. Combining with PT-141 is used by some providers for dual mechanism benefit — discuss with your physician.
